Unless the school's application instructions specify otherwise, there are no hard-and-fast rules about small formatting questions like these. Somewhere on the top of the page would be best. You should also include additional identifying information, such as your social security number, your date of birth, or an applicant ID number if the university has given you one. Repeat both your name and identifying number on each page, either in the header or the footer. If you have a title, we suggest putting it on the first page only, on a separate line after your name and identifying number, centered or left-justified.
